Sometimes it helps to put a number to how you have been feeling. The brief questionnaires I use are private and there to support our conversation, never to reduce you to a score or a label.

What these measures are

These are short, well established questionnaires used across mental health care. Each asks about how you have felt over the past couple of weeks and takes only a few minutes. On their own they do not tell the whole story and are not a diagnosis: they are a snapshot, a way to make the invisible a little more visible so we can talk about it together.

Not everyone needs them, and they are always optional. There is no right answer, only your honest sense of the past couple of weeks. Some find a simple score grounding, especially when feelings are hard to put into words. Others prefer to skip them, with no bearing on the work we do together.

The measures I use

A few brief, widely used tools come up most often in the practice. Each one looks at a different area.

  • PHQ-9, a short check-in on low mood and depression
  • GAD-7, a quick read on worry and anxiety
  • MDQ, a screening questionnaire about mood patterns

Which of these we use, if any, always depends on what you are dealing with and what would actually be useful to see.

A number never tells the whole story, but it can start a good conversation.

How we use them in the work

Usually we complete one early on, to get oriented, and revisit it now and then to see how things are moving. Scores that drift downward, hold steady, or climb give useful information about what is helping and what needs a different approach. Seeing the trend laid out plainly, rather than trusting memory alone, can be reassuring in itself.

These questionnaires support the conversation and are not a diagnosis on their own, which is a matter for a licensed medical provider. If a measure points toward something needing medical attention, I say so plainly and help you find the right person. Everything you share stays private, held as part of your confidential record.
